Sport Registration is now open in FinalForms for the 2024-2025 School Year! If you have a child who intends to try out or participate in a sport this school year, please log into your FinalForms account and click +Add Sports/Activities next to your student's name and grade.
Additional OHSAA Forms will need to be completed after you have made your sport selections.

If your child decides not to try out or participate, you may remove the sport selection in FinalForms at any time before the registration window closes.
Instructions
STEP 1: Click Here
STEP 2: Select "Parent Login"
STEP 3: Next to your student(s) name, click "Incomplete Forms"
STEP 4: If your child is participating in a Co-Curricular Activity, please select all that apply.
If your child is not participating in any activities do not select anything and click "Skip, proceed to forms"
STEP 5: Now you will see your formset for the 2024-2025 School Year on the left side of the screen. You will need to complete all of the forms (students involved with a co-curricular activity will have a larger formset) .
STEP 6: Once forms have been completed, they will change from red, to green or yellow.
STEP 7: Upon completion of the last form, FinalForms will generate an email notification prompting your student to login using their desales.co email, register and sign all forms requiring their signature.
